Hopkinton Little League Calling All Coaches

All coaching applicants please see the following message from Hopkinton Little League:

HLL 2013 Volunteer Application - REQUIRED BY FEBRUARY 15, 2013; SSN Mandatory per LexisNexis (LexisNexis used to check National Sex Offender Database) (Form is posted below).

Anyone volunteering in any capacity and/or having repetitive access to children in the Hopkinton Little League program (e.g. coaching, assistant coaching, team parent, field work, board member, regular dog house worker, etc.) must completely fill out the following:

Little League Volunteer Application for check of Criminal Offender Record Information (CORI) and National Sex Offender Registry

Form must be completely filled out with ALL requested information and one copy of U.S. Government Photo ID, signed and returned to:

No other forms may be used, past applications will not be considered and applications for other town sports will also not be considered.

ALL VOLUNTEERS MUST BE APPROVED THROUGH THIS PROCESS PRIOR TO ANY INVOLVEMENT OR INTERACTION WITH THE CHILDREN PARTICIPATING IN THE HOPKINTON LITTLE LEAGUE PROGRAM.
